FAB-2291 chaincode tutorials

This changeset provides two tutorials for chaincode.
one for Ade the developer and one for Noah the network
operator.

removed cc-packaging-and-signing.rst as the content
has been incorporated into chaincode4noah.rst
removed sample code, moved to fabric-samples
removed testing section as this is being done
separately
added devmode instructions
address binh and nick comments
