Checkout / Checkin procedures broken again

See notes in #1116, steps to replicate:

1. Create reservation for model with checkout procedures
2. Check out model and check off procedures
3. Notes indicate that procedures were not completed

Could this be a strong params issue? Also, the header for the checkin notes that follows missed checkout procedures appears to be missing a leading \n (see below):
