Need to fix EquipmentItem notes migration

Running into a bunch of issues while testing deployment since we coupled our migration to our Models... I'll quickly try to debug, and if it doesn't work then I'll just take out the post-migration code that auto-generated the equipment item histories just so we can move past it.

Ok, after looking some more at this we're going to refactor this migration to actually be written correctly. This means:

• Transactionalize the whole thing so that if the data updates fail, then the schema changes get rolled back
• Decouple this code from our models - write raw SQL for everything (worry less about eager loading simply because this only runs once)

In general, rewriting migrations is a bad idea, but since a) this has never been run in production to our knowledge and b) we're not modifying the schema change that results, it should be safe. This is very urgent so I'm hoping to have a PR open by the end of the week.

Pretty close here, the migration now runs successfully. I ran into a separate issue - there are a number of users with duplicate e-mails in the database, meaning that password authentication cannot be used without significantly cleaning it up. Just checking to see that everything works if I run the migrations with CAS_AUTH set.
