Requests: Markdown field for admins to ask patrons for the deets

Because each setup is unique and admins will want to know different things in considering whether or not to approve a given reservation request, we should allow admins to set a Markdown-formatted textbox on the New Request page to ask for details from patrons of why they're requesting special exemption from standard guidelines.

• Add column to appconfigs for said message
• Add default message to lib/setup_application
• Add field to edit the field in the appconfig/_form
• Display said message in request view

Implemented in branch 746_markdown_field

Waiting on the protected_attributes branch to be merged, since the appconfig whitelist of attributes will need to be edited before the functionality will be complete

EDIT Also, why wasn't this closed when we merged in the PR?

I think it should be pretty easy, since it doesn't touch the reservations controller at all or anything.
