Add default version string

If you didn't check out the tags during deployment (e.g. Heroku) there's no version number. We should figure out how to make sure that happens or, if it's not really feasible, add a default string.

Added function to environment.rb that sets the APP_VERSION to the number on the first line of CHANGELOG.md that has ### v
