Set up first user as superuser in `app:setup`

We should do this so that during initial deployment each instance gets a superuser. Also, let's get Casey added as a superuser to all instances so that he can add us :-)

I've discussed the second part w/ Mike and we should have that done in the next two weeks. I'll quickly update setup.rb to modify the role of the first user.
